Looking for the above. 

Where’s the best place to buy some pattern plates?

Any Steel Stockholder  or you could try Lyalvale Express if cardboard will suffice.

 
Most big shooting schools keep the printed card ones with a sort of dartboard counting grid  or dartboard and pheasant printed on them , about £0.50 to a £1 each . Alternatively if you have a mate who works in a warehouse ask them to “ aquire  “ cardboard pallet squares and draw your own 30” circle on them .

Do Husthwaite Pattern plates look like score cards with alterations on them?
Yes seen a few over the years. Couldn’t say I’ve ever been at a shoot and not seen one. The rumours usually started by spineless toss pots that you find making fake accounts on forums etc 
 

as for pattern plates of your not choke checking they really are pointless

The choke references supplied by Perazzi and Beretta are quite adequate for me.  I use the pattern plate to set up the gun's adjustable stock so as to get the POI that works for me.
