I did see a gun which bulged with a home load. Not as catastrophic as the gun in the link but it did blow the rib off. 

The guy who's gun it was loaded a cartidge with the  wrong primers for the powder used. 

 
The other dealer we usually go to (mostly for ammo) has a shotgun standing behind the counter with the end blown out. Wrong load, got caught in the choke. Luckily the guy was ok.

He covered all of this when I had my safety lesson with him (a pre-requirement for getting your license here) and it frightened the life out of me.

An older gentleman who came to our last shoot asked if I was shooting a 20g. I just told him I didn't want to risk mixing the ammo up, although I felt a little insulted he thought a girl couldn't handle a 12g!
